Independent medical examination -
Health Care Providers Removed from the Director's Authorized List

On May 13, 2009 the following providers were removed from the authorized independent medical examiner list because they did not have current or active licenses with their licensing board. No complaints or discipline from the Workers' Compensation Division were involved in these actions.

Oregon:
Stephen M. McCarthy, PT
Carin E. Rittby-Kane, PT
Candy M. Runnoe, PT
Howard W. Shoemaker, III, MD

Arthur T. Walker, DC


Washington:
Linda R. Alexander, PT
   
On May 20, 2009 Glenn T. Gerhard, MD and Allan R. Wilson, MD were removed from the authorized independent medical examiner list. The Workers' Compensation Division was notified of the death of both providers. No complaints or discipline from the Workers' Compensation Division were involved in this action.
   
On May 20, 2009, the Workers' Compensation Division received notice that James M. Watson, MD has retired. Dr. Watson has been removed from the authorized independent medical examiner list. No complaints or discipline from the Workers' Compensation Division were involved in this action.
 
On October 17, 2008, the Workers' Compensation Division received notice that Leroy Caspersen, MD has retired effective immediately. Dr. Caspersen has been removed from the authorized independent medical examiner list. No complaints or discipline from the Workers' Compensation Division were involved in this action.
   
Donald Peterson, MD was removed from the authorized independent medical examiner list effective September 30, 2008. The Workers’ Compensation Division was notified of Dr. Peterson’s death by his office. No complaints or discipline from the Workers’ Compensation Division were involved in this action.
   
On June 30, 2008, Heather M. Miadowicz, PT was removed from the authorized independent medical exam list effective March 31, 2008, because her license to practice physical therapy in Oregon "lapsed" on that date. No complaints or discipline from the Workers' Compensation Division were involved in this action.
   
On June 30, 2008, Norman D. Rabin, DC was removed from the authorized independent medical exam list effective April 30, 2008, because his license to practice chiropractic medicine in Oregon was not renewed. No complaints or discipline from the Workers' Compensation Division were involved in this action.
   
John Carter, MD was removed from the authorized independent medical exam list effective March 7, 2008, at the doctor's request. No complaints or discipline from the Workers' Compensation Division were involved in this action.
 
Thomas D. Caruso, DO was removed from the authorized independent medical exam list effective December 31, 2007, because his license to practice medicine in Oregon "lapsed." No complaints or discipline from the Workers' Compensation Division were involved in this action.
John S. Corson, MD was removed from the authorized independent medical exam list effective January 10, 2008, because he retired his license to practice medicine in Oregon effective that date. No complaints or discipline from the Workers' Compensation Division were involved in this action.
Joseph H. Eusterman, MD was removed from the authorized independent medical exam list effective January 1, 2008, because he retired his license to practice medicine in Oregon. No complaints or discipline from the Workers' Compensation Division were involved in this action.
Wesley G. Schooler, MD was removed from the authorized independent medical exam list effective December 31, 2007, because his license to practice medicine in Oregon "lapsed." No complaints or discipline from the Workers' Compensation Division were involved in this action.
Stephen C Zinsmeister, MD was removed from the authorized independent medical exam list effective December 31, 2007, because his license to practice medicine in Oregon "lapsed." No complaints or discipline from the Workers' Compensation Division were involved in this action.
